Objectiu de la convocatòria
The aim of this call is to solve undiagnosed rare genetic diseases and to address complex, multifactorial rare non-genetic diseases by identifying causative variants in patients with no molecular diagnosis after prior genetic or genomic testing and providing diagnostic clarity for conditions of unknown or mixed pathogenesis.

Suggested focus areas:
Functional validation to classify variants of uncertain significance (VUS) and increase the diversity of functional genomics research, or validation of candidate VUS to improve outcomes for a broader range of patients using in silico, in vitro or animal model systems (e.g. CRISPR modified cells, iPSCs, organoids).
Use of multi-omics or integrative methods (e.g. transcriptomics, epigenomics) to resolve ambiguous or complex variants.
New tools/methodologies not yet validated in clinical setting, including biostatistics, advanced bioinformatics, and mathematics approaches (e.g. variant effect predictors, AI-based annotation platforms).
Systems biology and disease mechanism modelling.
Integration of clinical, environmental, lifestyle and sensor-derived data.
Development of knowledge graphs or disease maps to link phenotypic and mechanistic insights.
Use of advanced AI and modelling tools (graph ML, probabilistic causal models).

Requisits
Joint proposals may include partners from academia, the clinical/public health sector, private for-profit and non-profit organisations, and patient advocacy organisations (PAOs).
Projects must be transnational and include four to six eligible partners from at least four different participating countries, with at least two from different EU Member States or Horizon Europe Associated Countries. In specific cases, the number of partners can be increased to eight.
A consortium can include a maximum of two partners requesting funding from the same country. There is no limit of collaborators per country, if their participation is justified.
Consortia should include and actively engage at least one patient partner. Patient partners, requesting funding or not, do not count toward the total.
Each partner is funded by its national or regional funding agency, following its own eligibility and financial rules. Investigators from HMRIB must apply for funding to the Instituto de Salud Carlos III (ISCIII).
Double funding of the same concept is not allowed.
PIs must hold a PhD, may participate in only one project proposal per call, and those affiliated with an IIS can apply through their IIS.
Only one PI per beneficiary institution may be funded within the same proposal.
PIs involved in an ongoing International Collaboration (PCIN) project of the same initiative ending after 31 December 2026 are not eligible unless they serve as coordinator in either the new or ongoing project.
Spanish research groups conducting a clinical trial must include a representative from their SCReN or ECRIN-ERIC node in the team.
A pre-eligibility form is mandatory: PIs must send their CVA-ISCIII form by email before the pre-proposal submission deadline to [email protected] and [email protected], indicating her/his full name and proposal acronym in the email subject line.
Full proposal applicants selected requesting funding from ISCIII must submit a national application.

Dotació
A maximum of two different partners requesting funding from ISCIII may participate in the same project proposal.
- Up to €220,000 per project if acting as partner
- Up to €275,000 per project if acting as partner with other partner
- Up to €300,000 per project if acting as coordinator
- Up to €400,000 per project if acting as coordinator with one partner

Durada
From 24 months to 36 months.

AVÍS IMPORTANT
The call is launched at the European level, but each partner is funded by its own national agency and must comply with the corresponding national rules. Due to the call's complexity, applicants are strongly advised to contact the Research Grants and Contracts Office early and to carefully review the call text and guidelines before starting their application.
